Medication Management

We’re here to help.

Medication management is just another name for a strategy designed to ensure that all of your medications are administered properly so they work as they should.

  • Prevents over- or underdosing
  • Ensures that medications are being taken at the correct times, without missing doses.
  • Helps patients adhere to prescriber instructions for all medications.

RxOne Pharmacy Advanced Care (PAC)

Pharmacy Advanced Care allows your pharmacy team to meet with you, one-on-one, to build a relationship and create a plan for the most efficient and effective medication management. With Pharmacy Advanced Care, your pharmacist works with your doctors so that everyone’s on the same page. We’ll also check in with you regularly to confirm that everything’s going well.

Would I benefit from medication management?

Anyone who takes prescription medications can benefit from medication management. It’s a great way to keep yourself safe and get the maximum therapeutic benefits from your prescribed medications. If that sounds good to you, feel free to ask about it next time you stop by!

How it Works

One-on-one meeting

A care team consisting of your pharmacist and prescribers will meet with you and compile a list of the medications you’re taking in order to move forward with everyone on board.

Making a plan
It’s often important to take certain medications at specific times or day or with food. We offer adherence packaging, which is organized by dose, day of the week, and time they should be taken. This can make it much easier to keep track of everything so you don’t miss any doses. Your team will work with you to figure out an adherence plan that’s perfect for you.
Checking In
We call each new patient 48 hours after medication delivery, 14 days after refills, and 25 days after refills. This helps us make sure that you’re not experiencing any negative side effects, and that things are proceeding as they should. Having a dedicated care team that’s advocating for you gives you the peace of mind that comes with knowing you’re being safe and responsible about your prescribed medications.
